ABSTRACT

Se presentó un caso de enfermedad trofoblástica tipo mola invasiva. El diagnóstico se sospechó con la ecografía del primer marcador genético, se realizaron dos legrados terapéuticos, el segundo de ellos, sugirió el diagnóstico de mola invasiva. Se realizó tratamiento quirúrgico (histerectomía total) con previa administración de quimioterapia con metotrexato sistémico. La biopsia confirmó el diagnóstico de mola invasiva. La mola invasiva es una de las formas de presentación de la enfermedad trofoblástica gestacional, es una condición infrecuente, en que la proliferación de células del trofoblasto y sincitiotrofoblasto penetran o incluso perforan la pared uterina, producen destrucción local pudiendo invadir el tejido parametrial y los vasos sanguíneos viajando dentro de estos, aunque no se han reportado verdaderas siembras metastásicas en otros órganos. La evolución clínica se realizó en consulta externa. Transcurrido dos años la evolución es satisfactoria, el tratamiento efectivo y bien tolerado por la paciente(AU).


A case of trophoblastic disease, of the invasive mole type was presented. The diagnosis was first suspected with the ultrasound of the first genetic marker. Two therapeutic curettages were performed, the second of which suggested the diagnosis of invasive mole. Surgical treatment (total hysterectomy) was performed with prior administration of chemotherapy with systemic methotrexate. The biopsy confirmed the diagnosis of invasive mole. Invasive mole is one of the forms of presentation of gestational trophoblastic disease. It is a rare condition in which the proliferation of trophoblast and syncytiotrophoblast cells penetrate or even perforate the uterine wall, produce local damage and can invade the parametrial tissue and blood vessels, traveling within these. True metastatic seeding in other organs has not been reported. The clinical evolution was carried out in an outpatient clinic. After two years the evolution is satisfactory, the treatment effective and well tolerated by the patient(AU).


Foi apresentado um caso de doença trofoblástica invasiva por mola. O diagnóstico foi suspeitado com a ultrassonografia do primeiro marcador genético, foram realizadas duas curetagens terapêuticas, sendo que a segunda sugeriu o diagnóstico de mola invasiva. O tratamento cirúrgico (histerectomia total) foi realizado com administração prévia de quimioterapia com metotrexato sistêmico. A biópsia confirmou o diagnóstico de mola invasiva. A mola invasiva é uma das formas de apresentação da doença trofoblástica gestacional, é uma condição infrequente, em que a proliferação de células trofoblásticas e sincitiotrofoblásticas penetram ou mesmo perfuram a parede uterina, produzem destruição local e podem invadir o tecido parametrial e vasos sanguíneos dos vasos. viajando dentro deles, embora verdadeira semeadura metastática em outros órgãos não tenha sido relatada. A evolução clínica foi realizada em ambulatório. Depois de dois anos, o a evolução é satisfatória, o tratamento é eficaz e bem tolerado pelo paciente(AU).

ABSTRACT

OBJECTIVES: Doppler ultrasonography can be used to assess neoangiogenesis, a characteristic feature of postmolar gestational trophoblastic neoplasia. However, there is limited information on whether uterine artery Doppler flow velocimetry parameters can predict gestational trophoblastic neoplasia following a complete hydatidiform mole. The purpose of this study was as follows: 1) to compare uterine blood flow before and after complete mole evacuation between women who developed postmolar gestational trophoblastic neoplasia and those who achieved spontaneous remission, 2) to assess the usefulness of uterine Doppler parameters as predictors of postmolar gestational trophoblastic neoplasia and to determine the best parameters and cutoff values for predicting postmolar gestational trophoblastic neoplasia. METHODS: This prospective cohort study included 246 patients with a complete mole who were treated at three different trophoblastic diseases centers between 2013 and 2014. The pulsatility index, resistivity index, and systolic/diastolic ratio were measured by Doppler flow velocimetry before and 4-6 weeks after molar evacuation. Statistical analysis was performed using Wilcoxon’s test, logistic regression, and ROC analysis. RESULTS: No differences in pre- and post-evacuation Doppler measurements were observed in patients who developed postmolar gestational trophoblastic neoplasia. In those with spontaneous remission, the pulsatility index and systolic/diastolic ratio were increased after evacuation. The pre- and post-evacuation pulsatility indices were significantly lower in patients with gestational trophoblastic neoplasia (odds ratio of 13.9-30.5). A pre-evacuation pulsatility index ≤1.38 (77% sensitivity and 82% specificity) and post-evacuation pulsatility index ≤1.77 (79% sensitivity and 86% specificity) were significantly predictive of gestational trophoblastic neoplasia. CONCLUSIONS: Uterine Doppler flow velocimetry measurements, particularly pre- and post-molar evacuation pulsatility indices, can be useful for predicting postmolar gestational trophoblastic neoplasia.

ABSTRACT

La mola invasiva es poco frecuente y se caracteriza por la excesiva proliferación trofoblástica y penetración del trofoblasto dentro del miometrio, pero sin tendencia hacia la diseminación metastásica. El objetivo del trabajo es presentar un caso diagnosticado de neoplasia trofoblástica gestacional, tipo mola invasiva. Se realizaron estudios complementarios como: ecografía transabdominal, determinaciones de la fracción beta de la gonadotropina coriónica humana y una biopsia endometrial por legrado uterino. El resultado fue embarazo molar. Al ingreso le fue administrado metotrexato sistémico 25 mg intramuscular y ácido folínico un ámpula intramuscular de 15 mg diariamente, ambos durante cinco días. Posteriormente, a los 20 días se realizó histerectomía total abdominal sin anexectomía. La evolución clínica fue por consulta externa. Además, se realizó: radiografía de tórax, ultrasonografía de pelvis, dosificación de gonadotropina coriónica humana en su fracción beta, hemograma completo, tiempo de coagulación, sangramiento y transaminasa glutámico pirúvica evolutivas. En el manejo de la mola invasiva el tratamiento médico combinado con el quirúrgico fue efectivo y bien tolerado por la paciente.


Invasive mole is an infrequent condition characterized by excessive trophoblastic proliferation and trophoblast penetration into the myometrium, without a tendency to metastatic dissemination. The purpose of the study is to present a case diagnosed with gestational trophoblastic neoplasia of the invasive mole type. The following complementary tests were performed: transabdominal echography, human chorionic gonadotropin beta subunit determinations, and endometrial biopsy by uterine curettage. The result was molar pregnancy. Upon admission the patient was administered systemic methotrexate 25 mg intramuscularly, as well as an intramuscular ampoule of folinic acid 15 mg daily, both during five days. Total abdominal hysterectomy without anexectomy was performed 20 days later. Clinical evolution was followed up on an outpatient basis. The following tests were also performed: chest radiography, pelvic ultrasonography, human chorionic gonadotropin beta dosage, complete blood count, clotting time, bleeding and evolutive glutamic-pyruvic transaminase. During management of the invasive mole, the combination of clinical and surgical treatments proved effective and was well assimilated by the patient.

ABSTRACT

A gravidez molar, espectro benigno da doença trofoblástica gestacional, representa uma complicação obstétrica da primeira metade da gestação. De maneira geral, cursa com sintomatologia exuberante: hemorragia, útero aumento para a idade gestacional, cistose ovariana e pré-eclâmpsia precoce; chegando mesmo a graves situações clínicas de hipertireoidismo e insuficiência respiratória.Todavia, devido ao diagnóstico precoce da mola hidatiforme, ainda no primeiro trimestre, mercê da ultrassonografia, houve uma importante redução na ocorrência desses sintomas; contribuindo para que a condução desses casos excepcionais ficasse confinada aos Centros de Referência. É objetivo desse artigo apresentar ao obstetra brasileiro uma revisão das complicações clínicas da gravidez molar, atualizando-o no diagnóstico precoce e tratamento dessas condições clínicas que podem ser potencialmente ameaçadoras à vida da gestante e de seu concepto.(AU)


The molar pregnancy, in the benign gestational trophoblastic disease spectrum, represents an obstetric complication of first half of gestation. In general, runs with exuberant symptoms: bleeding, uterus increase for gestational age, ovarian cistose and early preeclampsia; even serious clinical situations of hyperthyroidism and respiratory failure. However, due to the early diagnosis of hydatidiform mole, still in the first trimester, through ultrasound, there was a significant reduction in the occurrence of these symptoms; contributing so that the conduct of these exceptional cases could be confined to the centers of reference. The purpose of this article is to present to the Brazilian obstetrician a review of clinical complications of molar pregnancy, updating it in the early diagnosis and treatment of clinical conditions that can be potentially threatening to the life of the pregnant woman and her fetus.(AU)

Introducción: La incidencia de gestación ectópica es 20/1000 embarazos. La mola hidatiforme ocurre en 1/1000 embarazos. Los casos de mola hidatiforme en embarazos ectópicos albergados en las trompas de Falopio constatados en la literatura mundial son excepcionales. Objetivo: Presentamos un caso clínico de embarazo ectópico molar tubárico y revisión de la literatura. Conclusión: La enfermedad trofoblástica gestacional asociada a embarazo ectópico es un entidad extremadamente rara que debe tenerse en cuenta por su trascendencia.


Introduction: The incidence of ectopic pregnancy is 20/1000 pregnancies. The hidatidyform mole occurs in about 1/1000 pregnancies. The cases of hidatidyform mole in ectopic pregnancies located in the Fallopian tube collected in world literature are exceptional. Objective: We present a clinical case of an ectopic molar pregnancy in the tubes and a literature review. Conclusion: The trophoblastic pregnancy disease associated to an ectopic pregnancy is an extremely rare entity that should be considered because of its transcendence.

Introducción: el término enfermedad trofoblástica gestacional es una anormal proliferación del trofoblasto, que desde el punto de vista histológico se clasifica en mola hidatiforme, mola invasora, coriocarcinoma y tumor del sitio de implantación del trofoblasto. Objetivo: describir las características clínicas y ultrasonográficas de una mola hidatiforme parcial o mola embrionada. Paciente: se presenta un caso de 24 sem de gestación con el diagnóstico ultrasonográfico de mola hidatidiforme parcial. Resultados: se realizó cariotipo fetal y determinación de niveles de gonadotropina coriónica en suero materno. Se hallaron defectos congénitos en el feto, por lo que se ofreció asesoramiento genético a la pareja, que optó por la interrupción de la gestación. Conclusiones: el diagnóstico prenatal fue confirmado por la anatomía patológica


Introduction: the term gestational trophoblastic disease is an abnormal proliferation of the trophoblast, which is histologically classified as hydatidiform mole, invasive mole, choriocarcinoma, and tumor trophoblast implantation site. Objective: to describe the clinical and ultrasonographic partial hydatidiform mole or mole embrionada. Patient: we report a 24- week - prenancy case with ultrasonographic diagnosis of partial hydatidiform mole. Results: fetal karyotype was performed and levels of chorionic gonadotropin in maternal serum were determined. We found congenital defects in the fetus, so genetic counseling was offered to the parents, who chose ending this pregnancy. Conclusions: the prenatal diagnosis was confirmed by pathology

ABSTRACT

OBJETIVO: Determinar a frequência de mola hidatiforme em tecidos obtidos por curetagem uterina. MÉTODOS: Estudo transversal, prospectivo e descritivo que incluiu pacientes submetidas à curetagem uterina por diagnóstico de aborto ou mola hidatiforme cujo material obtido foi encaminhado para exame anatomopatológico. Foram excluídas aquelas que não aceitaram participar da pesquisa, recusando-se a assinar o Termo de Consentimento Informado Livre e Esclarecido. Foram analisadas as seguintes variáveis: achados anatomopatológicos, idade, raça/cor, número de gestações e abortos prévios, idade gestacional no momento do diagnóstico, níveis séricos quantitativos da fração beta da gonadotrofina coriônica humana e achados ultrassonográficos. As variáveis foram empregadas para a verificação com o diagnóstico histológico, considerado o padrão-ouro. Os dados foram armazenados e analisados no software Microsoft Excel® e no programa Epi-Info, versão 6.0 (STATCALC) e os resultados apresentados como frequência (porcentagem) ou média±desvio padrão. Para a associação entre variáveis qualitativas foi usado o teste do χ², e admitiu-se significância estatística quando p<0,05. RESULTADOS: No período, foram realizadas 515 curetagens, das quais 446 compuseram a amostra. A frequência de mola hidatiforme foi de 2,2% (dez casos). A média de idade das pacientes com mola foi 31±10 anos, a maioria era da raça branca e multípara e não tinha antecedente de aborto prévio, mas não houve associação significativa entre essas variáveis. A perda gestacional foi precoce nas pacientes com e sem mola, e as queixas mais comuns em ambos os grupos foram: sangramento vaginal e dor em cólica em hipogástrio. A dosagem sérica quantitativa da fração beta da gonadotrofina coriônica humana foi obtida em 422 casos (413 sem mola hidatiforme e 9 com mola hidatiforme). Os níveis do hormônio foram superiores a 100.000 mUI/mL em 1,9% das pacientes sem mola hidatiforme, e em 44,45% das pacientes com a doença (p=0,00004). Todas as pacientes com esse nível de hormônio tinham suspeita ultrassonográfica de gestação molar e uma delas apresentava também suspeita clínica. Trezentas e trinta e três pacientes foram submetidas a exame ultrassonográfico. Das pacientes com achados ultrassonográficos sugestivos de gestação molar, houve confirmação diagnóstica em cinco (41,7%) casos. Os outros sete (58,3%) eram falso-positivos. Houve associação significativa entre achado ultrassonográfico suspeito de gestação molar e confirmação da doença pela análise anatomopatológica (p=0,0001). Em metade dos casos de mola hidatiforme não havia suspeita da doença pelo quadro clínico, níveis da fração beta da gonadotrofina coriônica humana ou achados ultrassonográficos. CONCLUSÕES: A frequência de mola hidatiforme é baixa e a doença pode não ser suspeitada pelo quadro clínico, pela ultrassonografia e pelo nível sérico da fração beta da gonadotrofina coriônica humana, exigindo análise anatomopatológica dos tecidos obtidos pelo esvaziamento uterino para o seu diagnóstico.


PURPOSE: To determine the frequency of hydatiform mole in tissues obtained by curettage. METHODS: A cross-sectional, prospective and descriptive conducted on patients who underwent curretage due to a diagnosis of abortion or hydatiform mole whose material was sent for pathological examination. We excluded women who did not accept to participate and refused to sign the free informed consent form. We studied the following variables: pathological findings, age, race, number of pregnancies and previous abortions, gestational age at diagnosis, quantitative serum beta fraction of human chorionic gonadotropin and ultrasound findings. The data were compared to the to histological diagnosis, considered to be the gold standard. Data were stored and analyzed in Microsoft Excel® software and the Epi-Info program, version 6.0 (STATCALC) and the results are presented as frequency (percentage) or mean±standard deviation. The χ2 test was used to determine the association between qualitative variables and the level of significance was set at p<0.005. RESULTS: A total of 515 curettage procedures were performed, 446 of which comprised the sample. The frequency of hydatiform mole was 2.2% (ten cases). The mean age of the patients with a mole was 31±10 years, most patients were white and multiparous and had no history of previous abortions, but there was no significant association between these variables. The pregnancy loss occurred early in patients with and without a mole and the most common complaints in both groups were vaginal bleeding and cramps in the lower abdomen. Quantitative determination of human chorionic gonadotropin was performed in 422 cases (413 with and 9 without a hydatiform mole). The levels of the hormone were higher than 100,000 mIU/mL in 1.9% of the patients without a hydatiform mole and in 44.45% of the patients with the disease (p=0.00004). All patients with this hormonal level had an ultrasound suspicion of hydatiform mole and one of them also had a clinical suspicion. A total of 333 patients underwent ultrasound examination. Of the patients with sonographic findings suggestive of molar pregnancy, there was confirmation in five (41.7%) cases. The other seven (58.3%) were false positives. A significant association was found between ultrasound suspected molar pregnancy and disease confirmation by histopathological analysis (p=0.0001). In 50% of cases of hydatiform mole there was no suspicion of the disease according to clinical signs and symptoms, levels of beta fraction of human chorionic gonadotropin or sonographic findings. CONCLUSIONS: The frequency of hydatidiform mole is low and the disease may not be suspected by clinical examination, ultrasonography or the serum level of the beta fraction of human chorionic gonadotropin, requiring pathological examination of tissue obtained by uterine evacuation for diagnosis.

ABSTRACT

Se presenta un caso de mola hidatiforme parcial con feto vivo en la sem 32 de la gestación diagnosticado y tratado en el servicio de Obstetricia del Hospital Carlos M de Céspedes, en Bayamo, Cuba, además se realiza una revisión sobre el tema en la literatura nacional y extranjera. La coexistencia de una mola hidatiforme parcial y un feto vivo es una entidad muy poco frecuente, clásicamente se ha asociado a un alto riesgo de aborto espontáneo, muerte intrauterina, parto pretérmino, preeclampsia, hipertiroidismo y malignización. Está caracterizada por ser el resultado de una triploidía diándrica y por la presencia de cambios hidatiformes progresivos lentos con capilares vellosos funcionantes, que afectan solamente a algunas de las vellosidades; se asocia con un feto o embrión anormal identificable (vivo o muerto), membranas o eritrocitos fetales. El mal pronóstico de la gestación y los riesgos maternos generan un dilema sobre la terminación inmediata del embarazo o no. En el caso presentado se realizó cesárea al momento del diagnóstico con buen resultado materno y perinatal


This is the case of a partial hydatid mole with a live fetus at thirty two weeks pregnancy diagnosed and treated in Obstetrics Service of Carlos Manuel de Céspedes Bayamo, Cuba and a review on this subject in the national and international literature. Coxistence of this type of above mentioned mole is a uncommon entity, usually has been associated with a high risk of spontaneous abortion, intrauterine death, preterm labor, pre-eclampsia , hyperthyroidism and malignancy. Its characterization is the result of a diandric triploidy and the presence of progressive and slow hydatiform changes associated with an identifiable abnormal fetus or embryo (live or death), membranes or fetal erythrocytes. The poor prognosis of pregnancy and the mother risks lead to a dilemma on he immediate or not termination of pregnancy. In present case at diagnosis a cesarean section was carried out with a good maternal and perinatal results

Antecedentes: La incidencia de embarazo ectópico es 20/1000 embarazos y en la región cornual se presentan en el 2-4 por ciento. La mola hidatiforme ocurre en 1/1000 embarazos en EEUU y 1/500 en México. La presentación simultánea es muy rara. Se han descrito solamente 40 casos en la literatura médica, con una incidencia estimada de 0,04 por ciento de localización cornual. Caso clínico: Primigesta de 30 años, que cursando un embarazo de 8 semanas, acude de urgencia por presentar náuseas y vómitos de 1 día de evolución, con dolor abdominal y sangrado vaginal. Prueba de embarazo positiva. A la exploración física presenta dolor abdominal difuso y signos de irritación peritoneal. El ultrasonido transvaginal mostró imagen ecogénica multiquística intrauterina cercana a la región cornual, por lo que se sospecha embarazo molar. En la laparotomía exploradora se realizó resección cornual y posteriormente dilatación y curetaje de la cavidad uterina. La anatomía patológica confrmó el diagnóstico. Negativizó beta-hGC a las 7 semanas después de la cirugía.


Background: The incidence of ectopic pregnancy is 20 per 1,000 pregnancies. The cornual ectopic pregnancy is found in 2-4 percent. Hydatidiform moles occur in 1 per 1000 pregnancies in USA and 1 per 500 in Mexico. Thus, the incidence of the two occurring together is very rare. Only 40 cases have been reported in the medical literature, the incidence estimate is 0.04 percent. Clinic case: We analyzed a 30 year old woman patient case, gravida 1, with 8 weeks' gestation by last menstrual period, came at the Hospital Universitario Dr. José Eleuterio González, Monterrey, Nuevo León, with complaints of nausea and vomiting for 1 day and abdominal pain, and transvaginal bleeding. A cualitative pregnancy test was positive. Physical examination revealed diffuse abdominal tenderness and peritoneal signs. The transvaginal ultrasonography showed a multicystic echogenic mass within the uterus near cornual region consistent with a molar pregnancy. A cornual resection was performed followed by dilation and curettage. Pathologic diagnosis was confrmed. The monitoring of beta-hGC titers was negative 7 weeks after surgery.

Se revisaron 36 casos de embarazo molar resueltos con Aspiración Manual Endouterina (AMEU), en el Instituto Nacional de Perinatología, en el periodo comprendido del 1o. de enero al 31 de octubre de 1998. La Incidencia del embarazo molar en la institutción fue de 2.3 por 1000 embarazos. Casi dos terceras partes de los casos ocurrieron en mujeres con edades comprendidas entre 20 y 30 años. El factor de riesgo más relevante fue el antecedente de embarazo molar, que estuvo presente en el 44.5 por ciento de los casos. El promedio de edad gestacional fue de 12.6 semanas. Dentro de las técnicas anestésicas, se utilizó bloqueo epidural lumbar en el 86.1 por ciento de los casos y en los restantes anestesia general. El procedimiento de la AMEU fue realizado por diferentes cirujanos siguiendo los lineamientos descritos para técnica; en 12 mujeres (33.3 por ciento) se practicó legrado uterino instrumental (LUI) como complemento de la AMEU. Tres mujeres presentaron hemorragia de 1000 ml o más ameritando transfusión sanguínea una de ellas. Dos mujeres tuvieron retención de restos ovulares. Siete casos evolucionaron a enfermedad trofoblástica persistente. En más de 90 por ciento de los casos el reporte de histopatología "molar hidatiforme completa". El método de planificación familiar más utilizado después de la resolución del embarazo molar fueron los anticonceptivos orales combinados en 66.7 por ciento de los casos. Se concluye que la AMEU es un método seguro y efectivo para la evacuación del embarazo molar

ABSTRACT

Se define como histerectomía en bloque a la extirpación del útero grávido con su contenido gestacional in situ. Las indicaciones descritas para la relización de esta cirugía son los procesos neoplásicos, como la causa más frecuente; procesos sépticos, enfermedad trofoblástica y hemorragia secundaria a inserción placentaria anómala. Se recopilaron 12 casos de histerectomía en bloque desde el mes de enero de 1989 hasta diciembre de 1994 en el Instituto nacional de Perinatología, realizándose una investigación retrospectiva descriptiva. Se encontró un promedio de edad de 33.4 años. El número de gestaciones fue en promedio de 4.1. La edad gestacional se encontró entre las 9 y 29 semanas. Cuatro pacientes con antecedentes de 4 o más abortos. Dos casos informados con embarazo gemelar doble. Entre las complicaciones encontradas hubo tres casos de choque hipovolémico y uno con absceso de cúpula. La estancia hospitalaria en promedio fue de 5.3 días. La mortalidad materna fue de cero y la correlación histopatológica fue positiva en 100 por ciento de los casos. En la revisión de 5 años realizada, se encuentran 12 casos de histerectomía en bloque siendo la indicación más frecuente para su realización la enfermedad trofoblástica de repetición; en segundo lugar la hemorragia secundaria a inserción placentaria anómala, y en tercer lugar un proceso infeccioso grave. Las complicaciones observadas derivaron del compromiso hemodinámico de la paciente. Aunque la histerectomía en bloque es una cirugía poco realizada en la actualidad, debe estar dentro de las alternativas de tratamiento. Es un procedimiento que debe manejarse en hospitales de tercer nivel con tecnología y recursos humanos capaces de resolver las complicaciones que se derivan de esta cirugía

Comunicamos nuestra experiencia con 92 casos de mola hidatiforme estudiados en el servicio de patologia del Hospital Obrero numero 2 de la caja nacional de salud de Cochabamba, en un periodo de 22 anos. Analisamos su incidencia en la patologia quirurgica los aspectos anatomopatologicos, su relacion con el numero de embarazos, la distribucion segun edad los antecedentes ginecoobstetricos, la condicion socioeconomica de las pacientes, revisamos la literatura, los conceptos actuales y comentamos nuestras observaciones.

ABSTRACT

The epidemiology, clinical features and method of evacuating the uterus were reviewed in 30 patients with hydatidiform mole. The incidence was greatest in patients with blood group O and among young East Indians. There was no seasonal variation nor was there any progression to choriocarcinoma. Only one partial mole and one invasive mole were encountered. Evacuation was achieved chiefly by suction curettage and concomitant augmentation with a Syntocinon infusion
